The Department of Family Medicine is undergoing a survey. As part of this process, the survey committee is looking for feedback from the members of the department and from the broader medical school and university communities on its perceptions and experiences working within/with the department.

We are inviting external stakeholders to the department (e.g. those who interact with this department) to provide input via the following questionnaire, by听November 7, 2024:听

This questionnaire should take no longer than 5 minutes to complete and is completely confidential. Your identity cannot be linked to your responses and only aggregated results will appear in any reports or communications regarding the survey results. Please read each question carefully and choose the response that best applies to you. If you do not understand what is being asked, an item does not apply to you, or prefer not to respond to an item, please skip it.
